Referrals and Enrollment

All referrals must be completed by a mental health professional.

PRP Criteria

Adults 18+ years of age

Must meet priority population diagnosis criteria

Desire to participate in program and services on a voluntary basis

Maryland Medical Assistance

Self pay is available

Live within our catchment area or can self transport or use public transportation

Individual is not currently in need of a higher level of care, such as: inpatient care for mental illness, complex medical illness or active substance abuse

Supported Housing Criteria

Adults 18+ years of age

Must meet priority population diagnosis criteria

Must be able to function independently within the community

Individual is not currently in need of a higher level of care, such as: inpatient care for mental illness, complex medical illness or active substance abuse
