According to a report from the U.S. Department of Health Human Services, March 2012, up to half of all patients from psychiatric hospitals require readmission within 1 year of discharge.

Case management services

Medication education

Appointment coordination & support

Independent living skills

Development of effective coping skills

Person centered treatment planning

Peer support system

Community resource facilitation and entitlements
