Outreach Services

Our outreach services offer clients an opportunity to continue living successfully in the community while receiving individual support and case management from their rehabilitation coordinator. Our teams support clients to have better appointment management, and assist them in maintaining their independence. Outreach services may take place in the home, or in the community. Service areas include Baltimore County, Baltimore City, Montgomery County, and Prince Georges County.
